The "Recipient" is the potential awardee of tickets and travel contribution from The Foundation.
"The Applicant" is the person completing this form.

THE LIZ SOPER FOUNDATION OF CARE ENRICHING LIFE EXPERIENCE AWARDS

Terms and Conditions

PURPOSE
To enable individuals living with complex needs to enjoy recreational opportunities and enriching life experiences (of their choice) that are usually out of reach due to the complex needs of the individual and financial restrictions. 

REGULATIONS
The recipient will be 18 years or older at the time the application is submitted.

This Award is for Entry Tickets ONLY for the recipient and their support companion. All other associated costs e.g. transport, care and support payments, food etc are to be covered by the recipient/support companion/applicant.

The Recipient of an award may or may not be the applicant.

  1. Recipients of an award must be:

    1. New Zealand Citizens or Permanent Residents, residing in New Zealand.

    2. Persons with disabilities who are in need of care and support so they can participate in extracurricular activities.

  2. Applicants will be notified of the success of their application either way.

  3. Each recipient of a Liz Soper Foundation of Care Award (or their nominated support person) is required to submit photographs and supporting words within two weeks of completion of the activity.

On submission of your application you will view on screen an acknowledgment that we have received your application. Applications are not reviewed until after the closing date.
